Art. II. - AMBULANCE SERVICES AND EMERGENCY MEDICAL CARE |
§ 21-34. Operating requirements.
The parish shall not issue an ambulance operator's license unless and until the following requirements are fulfilled by the applicant. Continued fulfillment of these requirements is a condition of maintaining the ambulance operator's license.
(1)
The applicant executes an agreement, in a form approved by the parish attorney to indemnify and hold harmless the Parish of Webster, its officers and employees, and members and staff of the Webster Parish Communications District Board, from any liability arising out of the licensing of the applicant or the performance of the services provided under said license.
(2)
In addition, the applicant shall file with the Webster Parish Communications District Board a certificate of insurance evidencing the following insurance coverages and naming the Parish of Webster as an additional insured:
a.
General liability coverage with liability limits not less than five hundred thousand dollars ($500,000.00) per occurrence and five hundred thousand dollars ($500,0000.00) in the aggregate.
b.
Automobile liability coverage with liability limits not less than five hundred thousand dollars ($500,000.00) in combined single limits and five hundred thousand ($500,000.00) in the aggregate.
c.
Medical malpractice liability coverage with liability limits not less than five hundred thousand dollars ($500,000.00) per loss or proof of participation in the Patient's Compensation Fund.
d.
Such policy shall expressly provide that it may not be canceled, except after thirty (30) days' written notice to the Webster Parish Communications District Board. Additionally, the insurer shall be required to give notice of cancellation or non-renewal to the Webster Parish Communications District Board.
(3)
Ambulance operators who respond to emergencies in Webster Parish furnishing advanced life support services will operate under the supervision of a physician medical director using protocols approved by the parish medical society and having a written agreement with hospitals to furnish on line medical control.
(4)
Each vehicle to be used as an ambulance complies with the requirements of this article for an ambulance vehicle license.
